Inspection notes: "Observed buildup of dust and debris on shelves in walk-in cooler and on multiple pieces of equipment throughout kitchen (shelves, outside of mixers, etc). Observed buildup of grime/debris on floors and walls near pizza oven. Observed mops hung in position allowing for mop head to soil mop handle. Observed multiple pieces of equipment that are no longer functional and are not used (reach-in coolers, bulk ice machine). Observed numerous flies in unused bulk ice machine in kitchen."
